PC, no mods, new issue after September 22 update, I play with autonomy off. I think the fix so Sims would stop wearing only their first outfit has messed something up, because now my sims are constantly changing their outfits. I took them to the beach and put them each in a swimsuit. Each time I directed one to do an activity (swim, sunbathe, snorkel, sail on outrigger canoe) they would change into a different swimsuit. Also, after they got home, one Sim was already in her sleepwear, but when I sent her to bed, she changed into a new sleepwear outfit. It breaks the realism, the changing clothes sound gets really repetitive in a 5-sim household, I care what my sims wear and have usually chosen to dress them in a particular thing, and I shudder to think about the piles of clothes if I was using Laundry Day. Frankly I wish they wouldn’t automatically change clothes at all if autonomy is off.

Also having the same problem and what has become my personal pet peeve is related to the fact that I give Sims with any of the Snowy Escape skills a wintersports outfit in the cold weather category. I used to dress them in that specifically when going to the mountain and suddenly they change and go down the slope in their comfy sweater and I am just sitting there feeling like facepalming.

I noticed this as well but wasn't sure if I was just misremembering how much laundry generated. My Sim had over 30 piles. I'm also having to frequently tell them to start the washing twice.
Same especially with swimwear.
This has been happening for me too on PS5 after the update in September. Doesn't matter the action. If you already have them in athletic wear, they change into athletic wear again when you click on the workout equipment or "Go jogging." If they are going to bed and already in PJs, they'll change again before getting in bed. Which is also messing up the sleep and cuddle interaction and waking correctly because they are trying to change outfit again when waking. Riding a bike, the everyday outfit changes and so on. It definitely does make the laundry accumulate.
This is crazy, for every task I give them they change clothes, they even change swimsuit 3 times while getting into water. Right now my Sim plays basketball and changes clothes with every jump👀
Primarily the athletic & pajamas categories. They just keep changing info outfits I don't want them to wear.
I bought Adventure Awaits and Lovestruck last week. When I went into my legacy file afterwards I saw that my sims outfits were glitched. Half of 1 outfit and another are mixed together one one sims and on another my sims is wearing gloves and shoes that I didn’t put on him( and is only visible in live mode)
Same for me too. They constantly change their clothes. Right after this last update, it doesn't matter what season; for me, it's so random- any outfit from their wardrobe. Especially when they travel to a new location
I’ve noticed the two adult sims on my current save file will go to bed in a sleep category outfit but when they wake up and get out of bed they’re in one of their everyday outfits, usually one inappropriate for the weather (ex it’s warm and they come out of bed in a long sleeved outfit. It’s still an everyday outfit not a cold weather category outfit but it’s just plain weird). It seems especially prevalent on the woman rather than the man, which I also find odd. This particular file has the parents and four kids, one is a child, one is a toddler, and two are infants. So far none of the children are having the same bug.
I do have mods but it only started after the big patch in September.
