Have you tried deleting the localthumbcache.package file in your The Sims 4 folder? It stores images of sims for faster loading, but it can also cause old images to stick when they're not supposed to.
I've tried to delete the localthumbcache and localsimtexturecache files to see if it solves it but the issue still happens after opening the game.
