Sims standing in different location after reload

So I was having a birthday party for a toddler, and the toddler appeared right next to the buffet. I then made one of my sims give the toddler at piece of cake, the sim took the cake, went upstairs to 2nd store bathroom, I then made another sim give her (the toddler) a piece of cake, and this sim also went upstairs to the bathroom, even though the toddler was next to the buffet at 1st floor.

So I saved the game, went back to Main menu, and reloaded the game, when the game was loaded, I saw the toddler sitting on the potty in the 2nd store bathroom, and two sim standing next to her, each with a piece of cake.

There's similar threads to this. I'll check with a CM if they want to track this separately or as a new issue.

Could you please add thread tags for your mod usage in the mean time. If you have mods, remove them and test without.

Yes of course.
I play Sims 4 on desktop pc with no Custom Content and no Mods, and I have currently only the EA Backyard Stuff Pack.

Play on PC via EA app, no MOD/CC, Autonomy full. I save every single file with as save as, and start to play with Load Game.
I have sometimes experience this issue.
Just before save
Just after load game
This happen randomly both time when I queued action and not. But I feel like this issue often happen that I saved when invisible actions in the Sim's queue.
