This is happening to my sim too after the latest update. I used paint mode to add make up and a tattoo - I'm not sure I can remove these but will try. I have no mods or cc installed.
Just an update - removing the tattoos worked. Very disappointing that I had to do that - I've used the paint mode on every sim I've created since that feature came out. Not game to go into my saves because of this. I hope it gets sorted out soon.
