I'm having a similar issue but specifically with the hat while in a career outfit. I have one sim that is a cop (GTW) and she wears a brown baseball cap, and another in the Entertainer career (Johnny Zest) and he wears a fishing hat to work. All other outfits are unaffected. I've never modified his clothing or copied outfits, though I did copy makeup presets for the one that is a cop. She was previously a scientist and was wearing the same random hat. The rest of her career outfit is unaffected.
Edited to add: And just spotted sparkly high heels on Johnny Zest in his career outfit as well. So it's not just the hat anymore.
