I reloaded my save and both parents went to work but I made sure to feed the baby right before my sim walked out the door. Of course the baby gets hungry again but the butler didn't feed him until the second time she went back to him. Neither time did she change his diaper and she just leaves him laying in stink. It's hard to see but I tried to adjust the view so you can see the fumes wafting up. Posting the screenshots in chronological order.
This problem is not just with butlers, parents do it too. But when the butler is the only way the baby can get cared for, it is important that they actually do so. If the child has sensitive skin, leaving them sitting in this diaper probably would not end well either.
After I took these photos, she finally comes back and changes the baby THREE times. Then she goes off and shortly comes back and changes him like five times. Back to back. It just makes no sense.
