Are you playing on Pc? Sometimes a program like Equalizer Apo can cause this issue. This is a program to increase your Pc volume. If you have this program lower the volume gain to 12,13 or 14 dB.
I don't know anything else because i play on Xbox but maybe you should disable any equalizer, sound enhancements or something similar in your audio settings and see how it goes.
