Playing with a premade squad would eliminate half of the complaining on these forums to be honest.
However, the game ALLOWS solo queuing in a team-based game, so the developers should take that into consideration.
The easiest solution to this problem would simply to have only premade stacks enter a game and only solos enter a game. Do not make a mixed lobby of premade stacks and solos / duos.
Some versions of sabotage you listed are easily discoverable with computer code - If you're dropping and then don't land on solid ground and fall off the map *boom* sabotage = banned. Even if your sabotage was unintentional, its still sabotage. Are we to understand that only purposeful sabotage is being considered? From what I understand, the security personnel don't have live clips to watch of the reports and are just looking at data. Not sure how they would assess "intent" without visually watching something. Moreover, if they CAN visually watch a clip of the report, then teams dropping and dying immediately should be viewed as "intentional" especially if a teammate is rapidly pinging "enemy near" The Jumpmaster intentionally putting their team in harm's way should be seen as sabotage.
Nonetheless, I've never seen or heard of anybody getting banned for this category, and there are other things like "wallhacking" which aren't even reportable.
Getting rid of premade squads would increase the queue times.
Solo queue players getting into a premade however does not affect the queue times in any way whatsoever.
Banning someone for flying off the map could work, though if someone is having network issues it could cause them to fly off the map, thus causing a false trigger.
And yes we would be talking about purposeful sabotage as people can make mistakes, though you're correct that this would be near impossible to detect from just gameplay footage.
Typically these systems work by flagging a user if they get reported a lot for the same thing, which could indicate that they are blatantly trying to sabotage their teammates.
I've also heard that wallhacking should be reported under "gameplay exploit" though I doubt that's actually the case.
Think it's kinda weird too that this isn't really an option.
Either way, lots of players get banned for a lot of different things, those who know that were sabotaging will most likely not make a post about it and even if they do, numbers might be low.
The reason for their ban also might have a different reason in the mail they've received, so it might not paint a good image of what caused the ban.
