I feel like you've hit on almost every single reason why team-based games suck. As you said, it's not the servers, it's the people. Once in a while they're great, but mostly they're reckless and blame everyone but themselves for what goes wrong.
I don't play any foreign servers, so I can't speak to that part of the discussion, although like @pandareno1999 I watch European streamers once in a while. Same crap, different continent.
But here's what I have observed on the New York server: while most players are trash-talking gangstas, it's the women I've played with who consistently are polite, helpful, occasionally team-oriented, and often good communicators. When I get squadmates with gamertags that obviously belong to a female, I breathe a sigh of relief that I'll have at least one match without the toxic garbage that usually makes me mute before the game has even started.
