Does this seem to happen at a certain time of day, or a certain day? Also if you haven't already can you try the following to see if anything chages?
- Connection and Advance Connection Guides
- Change TCP/IP Settings
- Untick the options in LAN Settings (make sure nothing is checked under Proxy Server)
- Change DNS settings (You can use the public google DNS primary: 8.8.8.8, secondary: 8.8.4.4, unless there is a different DNS you would rather use)

To answer your questions and give you some more information.
1. I have done basic connection optimization steps.
2. What exactly should I change about TCP/IP?

4. I will change my DNS to googles and see if that helps.
But I also have been doing some research, and I can tell you some relevant information.
1. Many people in copenhagen with otherwise rock solid connections and ping have this issue
2. It is only in Apex Legends
3. It is only with the provider yousee.dk, affecting those with both coaxial and fiber connections
4. Yousee service has no other problems with other games, is only Apex specific. I have seen somebody has raised this issue to them, they replied that its unfortunate, asked if it could be the games servers. But it seems like this is only an interactive effect between Apex Legends network demands and Yousee network.
5. Connections to the same Apex server region will yield different results in different games, e.g. same server gives 20 ms ping in one game, and a steady 220 in the next game. My bother plays with me online, same region. He will have 20 ping, me too, and then the next game he will again have 20 ping, while I have 220. But when I ran a speedtest for my connection in that moment when my game showed 220 ping, I have 2ms ping to nearby data center
6. Changing Apex server region does not help
I am trying to figure out if it happens on specific days or at specific times, keeping a log for a while. For now what I can say is that its irregular, but very frequent. e.g. 1 bad game, 1 good game, 1 bad game.... or 10 good games in a row, then all of a sudden I cant get rid of 220 ping.
I can guarantee that in theory my internet is insanely reliable and rock solid. Speedtests to nearby servers usually give me 1 ping and never more than 3.

Yeah, it is Apex's servers just being annoying towards Copenhagen, a VPN fixes it, but still an annoying solution, because it worsens ping, and you have to remember to put it on
