I am seeing a number of Windows errors popping up here for various services. Can you try running the System File Checker tool to see if that's able to correct any of this for you?
I'd also suggest disabling any other overlays, in particular the Xbox Gaming overlay here. If that still continues, you may want to try a Clean Boot of your system.

If you had run the SFC tool prior to grabbing that DxDiag file, I'd try running it again to be safe. There are definitely some Windows services crashing that shouldn't be.
Aside from that, I'd also try going through the troubleshooting steps in this post, as the issues they helped resolve were extremely similar to what you're running into.
I changed from steam to origin, the game itself loads pretty fine it gets me into the lobby but when i want to ready up for the game it just says "loading "map name" constantly and nothing happens any tips on this one?
