EA_mplsnow same issue pls help. The page doesnt work...we need a way to contact TOS. the webform has no EMAIL US button.

Can you please confirm you're on the correct EA account? The penalty history tool and ban options will only populate if an action exists on the account you're signed into.
You can confirm by checking your purchase history and connected accounts for recent and relevant entries. If you're on the correct account and everything looks right, please send me a screenshot of what you're seeing via DM, and we can continue from there. Thanks!
