Thanks for giving it a try! When you've got the chance please try running through the basic connectivity troubleshooting steps from the EA Help website to see if that helps. If you're still having the same issues after that check out the PlayStation-specific steps as well.

Still the same, don't seem to have any connection issues, I tried uninstall/reinstall,, system is up to date, and account is linked, etc. I've gone through the list and everything seems correct,
There is no error message, it just seems to act like I'm not in a club when I'm in a club.

Hmm, that's really odd. You aren't seeing any errors pop up or anything before it seems to remove you from the Club?

It doesn't remove me from club, I attached photo of club screen and what I see before and after game.
I know I'm in club since it shows me in at the beginning and end screen of match, but when I go to the club tab it doesn't show me my clubmates, it just shows me the invite and create options. I've tried going into other clubs and same thing happens, It shows me in their club before and after match but I can't see club mates on the club tab.

No pop ups, errors, anything have come up.

I'm wondering if it's just a visual error of some kind in that case. A while back we were seeing similar situations where it would appear that someone was removed from the Club, but they actually weren't.
Do you have any other accounts you could use to sign into Apex Legends on your PS4? If so, would you be able to test this and see if the same thing is happening on other accounts? I am curious if this is specific to the account you're currently using.
