Do you have a video of it happening? There was a bug a few months ago which lead to movement issues, but this has since been fixed. 

It does sound like there may be a controller issue (sometimes they're DOA), do you have another one you can use to test? And do you get issues with any other games? I've had a few controllers die slowly and the issues can manifest to different degrees in different games.
