If that chat cut out during an active case it may take a bit to be resolved/closed before it'll let you open a new one right away.
When you tried an incognito window, did you sign back in before trying to create the case? If you open another incognito mode window, try selecting Continue without logging in when you're creating a case. You'll still be able to let the advisor know which account's having the issue, and that should let you get back in touch a bit faster.
