This is an issue with the game, not the console. I have don't every reset possible, including game re-install, and no fix has worked 100%.

Are you experiencing this on an OPEN NAT? You can find it under your Xbox Network Settings.

What is so odd is the inconsistency of the issue. Just to mention, my job is to troubleshoot classroom technology so I am good at trying it all, isolating elements that are not at fault and so on. I firmly believe you have a game bug.
I have 5 matches with no audio, then one match I have player audio, then 10 more matches I do not.
I have power cycled my unit, checked controller for firmware update, rebooted router, modem, went appear offline, online, reinstall game. Voice chat to speaker, voice chat to both, check in console mixer, toggle every combo of audio settings in game setting's menu. I have tried everything I can think of and still get less than 5% of my games to get chat audio.
Question: I assume, if I am seeing a voice icon appear on the bottom left of my screen, indicating voice is heard, that this is the game chat visual indicator recognizing audio. NOT party chat audio. Is that correct?I know the overlay for my xbox party voice chat location, this is not it. I am speaking to the game indicator for who is talking.

- What type of headset are you using? (model/brand)

Astro A-10, plugged directly into controller via 1/8" headphone connection. No mix amp.
Destiny 2 voice chat is flawless, fyi. As are any other game audio for me.The ports you link to, a few clicks through, lead me to the core xbox live ports, correct? They are all except 1863, which appears on your NHL 19 list, but not on the xbox list.
Is there a list for Apex?

I tried forwarding those extra ports and still unable to hear teammates. I do not see their microphones register as talking but I do see it type in chat that they can hear me on their end. I can see my Mic light up so I know they are talking. I believe when I play with people that are in parties, they are showing as talking all the time. I was not in a party. I even started a party and switched to game chat but it did not work. After leaving the party, I was also not able to hear them.
I have Astro A40 TR's using the Astro MixAmp through USB. Any other suggestions, I'd be glad to look into.

I have experienced this with the Microsoft branded headphones as well as the turtle beach set i just bought

The astros i use are simply headphone jacks with a mic. Like any iPhone headset. No mix amp in my situation.

i have 2 different pairs of turtle beach neither of them work with this game all other games are fine

Hey, Blueberry.
I too am experiencing this issue on a turtlebeach stealth 600 headset. It’s been going on since launch and never once worked. Xbox party chat works fine but in-game voice chat does not. I too can see mic symbols light up, even my own, but i cannot hear anyone. It is quite frustrating.

I'm playing with a Steelseries Arctis 3, and have an open NAT. Can hear friends in xbox party chat and in game chat. Still have never heard anyone that is not a friend.

I have open NAT and get no audio from my teammates with 3 different sets of headphones:
1) Astro A-10
2) Standard iPhone in-ear headphones
3) Polk audio gaming headphones
All 3 work perfect with any other game, just Apex that isn't working.

I have a Turtle Beach headset too. When I was playing with my friend and we were in our party chat, the microphone icon in game lit up as if someone was speaking. When went to game chat, the microphone icon would only light up when we were actually talking. You can test this by going into a party with your friend and switch from the party screen when you double-tap the Xbox home button. We'll provide an update around this as soon as possible.
