Xbox button>Profile & System>Settings>General>Network Settings> Is Nat Type Open ?
Xbox Nat Type should be open to avoid any disconnections.

Network Settings>Test network speed & statistics> do you have any packet loss? post your results
In game if you have 0-2% packet loss then it's fine.
In game I play Frankfurt server 90-98ms most times 0-2%packet loss.
While doing the test or playing the game check that you are the only one using the internet or you'll have big packet loss and lag especially if your connection is slow.
Also, have you tried to use Wired connection Ethernet cable?
Maybe Wifi is the problem.
