Time nudge jumping up in combat is usually due to the framerate dropping below 60fps.

Do you see 3 yellow rectangles in the top left corner of the screen? This indicates a framerate below 60fps.

Please post a screenshot with the in-game network performance graph on-screen to help with further troubleshooting. Having the framerate on-screen will also help if there's an option for that in the game menu.
