Possible Issue with Grenade Indicators Since Season 4

I'm not sure if this is an intentional change or a bug, but since the start of Season 4 I've noticed that I sometimes get killed by enemy grenades without ever seeing a grenade warning indicator.

Is anyone else experiencing this, or is it just me? It feels like the indicator is either inconsistent or sometimes doesn't appear at all.
