I died walking through a door and the game autosaved. It now loads the save and I can hear the audio of the main character dieing but I can only see the loading screen. It's just stuck on a loop.
Same thing happens when I launch the game it loads the last save and gets stuck in the loop with audio or the death sequence. I can't pause it or access the menu to load another save. ☹️

Okay there’s a way out of it but it’s a pain.
Important steps to follow:
Requires disconnecting controller after seeing the epilepsy warning and make sure you don’t have a bluetooth headset connected because it will make the ps5 overlay stick.Whenever you fail the attempt you’ll need to close the application and begin again.
I was able to get out of the death loop by disconnecting the controller, reconnecting ~3 seconds after hearing the flatlining sound, and pressing X right before being able to see the message “your controller is disconnected.”
If done correctly you will see the menu and be able to scroll down to load game. Be aware not to select the main menu option like I did or you’ll be thrown back into the death loop again.
It took me like 100 tries to get it right but it works and is easy after you get used to the strict timing.

I’m have the same issue. PS5 digital doesn’t seem like the system matters. Also on the newest version 1.000.004. Had received an invite from Sony for their beta program a day or two before this started happening. Revert back to regular consumer OS as an initial troubleshooting. That didn’t help.
Also tried clearing PS five cash and rebuilding the database either which helped.
I’m trying to do this one solution where it says to disconnect your controller, but my hold on the PS button, it exits out of the game, so I’m not sure of another way to disconnect a controller without that happening.
This is obviously a very annoying error as I’ve been dying for this game to come out with much anticipation and now it’s currently unplayable

To disconnect the PS5 controller don’t hold the PS button. Instead quick press the PS button and scroll to devices on the right and choose turn off controller.
and no, the system doesn’t matter. This is happening on all platforms including PC.
