Did you happen to romance Bellara on the bugged save?
I'm trying to find some common ground here since I did and I feel it might be a factor in the mechanics of the missing trophy/achievement. Usually with the other companions, it unlocks after their final conversation in the Lighthouse after the outing. Like their mini epilogues. So I assume it would pop not during the brazier lighting but at the chat you have after? But if related to the romance, then I may need to backtrack all the way to prior to the commitment and deny the relationship (the "since there's too much going on" pre-relationship breakup. T. T this was the hardest "nevermind let's stay friends" option of all the companions istg!)
I've tried reloading into the Lighthouse, fight some enemies, etc and nothing new happens. I was already at the point of no return since I was saving her final quest as last as she was my romance option this time, so no new quests to really do. She was also already level 10 by this since her and Volkarin are my favorite party members. Later on, I didn't let her do the ward in the finale so I can see the romance scene and still nothing (I assumed that her companion "ending" would just adjust towards the romance ending instead in case it was coded as an either/or flag).
