Can't Die on Hardest Difficulty Despite 'No Death' Disabled

I don't know why I can't die with the  combat options set as hardest difficulty (disabled the no death option an available only in the easy mode), I don't wear any gear that change this, I remember being defeated at the start but I can't now, anyone experienced this? Even if I stay in the middle of combat doing nothing and with the red screen 🤔
