Rook gets stuck doing heavy attacks over & over & over indefinitely.

Rook gets stuck doing heavy attacks over & over & over indefinitely. It doesn't stop unless I hit jump. Even after I jump it will start all over again if I hit any action button, even if I hit tab to access companion abilities. It will go on indefinitely if I let it. Even if I restart a saved game it instantly starts heavy attacks once the game loads. I have to completely shut down to windows desktop and completely restart the game with steam to get it to stop, and even then it eventually starts to do it again. This completely ruins all combat combat. Any help would be greatly appreciated. This completely ruins the game for me.
