Just to confirm, on the EA App, if you select Battlefield 4 > Manage > Manage Add-ons, do you see the five DLCs included within Battlefield 4 Premium there?
It looks like you purchased Battlefield 4 Premium, not the Premium edition, so there wouldn't be a new tile apart from your current Battlefield 4 tile.
