PS5 Multiple achievement Bugs - Tales of Kenzera: Zau

Act 3 achievement did not occur. When I was approaching the boss for Act 3 I died on approach but spawned in front of the boss. Beat the boss and did the escape. The quest marker for the Act 3 boss persists and the route is now closed off. 

Also missing Perk Up and Soulful even though I have all the trinket slots, and have completed all spirit trials completely. 

According to the game I have 100% completion but not these trophies mentioned.
