"DirectX function "GetDeviceRemovedReason" failed with DXGI_ERROR_DEVICE_RESET

I'm having this problem since before launch. When I alt-tab it usually crashes. When it's crashing it crashes my discord and EA app too. I already made a clean install for gpu driver, tried bunch of other things sadly issue still persists. I'm also using RivaTunerStatistics I thought that was the issue because anti-cheat might be blocking it but even after disabling it issue still persists.

FC 24 I was playing from few weeks, suddenly from today it's crashing every time with DirectX function getdeviceremovedreason failed with dxgi_error_device_reset. 

Attaching the screenshot and DirectX test result for your reference. Please help me to resolve the issue, as my PC is having minimum configurations for this game and it should not crash.

Please help as I am die-hard football fan, can't live without playing for a single day, help me out.

The game gives this error 5 minutes after opening and closes. This especially happens when we pause the game. so I lose a lot of points and I don't want to enter the game. I haven't been able to have a game night for a long time, I get angry and get caught up.
