Disconnect Frequently is Back, EAFC Loading Symbol of death

I've posted this issue before, and it was resolved for a solid 3 days where my ping was no longer 45 it was 28-30 consistently. 

Now I'm running into the same issue AGAIN, where any games I play if I see the EAFC loading symbol in my games i know I'm going to disconnect. 
Ping is gradually going back to 39-44 range. 

I don't have this issue with ANY other games. 

I'm running out of ideas as to why this is happening. Considering we/I pay top dollars for this, the bare minimum is to able to support consistency.
