As others have mentioned, and it is in my case, this can happen when a Created Player is in the squad of Career Mode. The player disappears from the squad which then causes an error when trying to open Squad Hub.
My workaround that I just tested out was releasing that created player before it disappears. In my case, I release him from my team after the last game of the season. Hope this solution works for you guys also
