Appleal Not Submitted on Ban Appeal

Whenever i try to create a ban appeal it hits me with the 'Case not Created', i have already tried it on many different devices and browsers, i have already cleared my cache, nothing works, icongnito window 

as you can see no matter what i do its always the same thing, please help me as i need to appeal this decision as i think it was false.
