I was onable to play live event matches

Hello 

For the past few days I haven't been able to play live event matches so I decided to Uninstall the fc moble app then reinstall now I can't even get past the start up screen its asking for my birth month and year to play 

Plz I don't want to lose my team I have add this account for over 5 year and I don't think am ready to start all over
