achievements are not unlocked

I just finished Mass Effect 2 via Steam, and I got a romance with Miranda and completed it on Insanity. When I did this, the respective achievements were not unlocked. I clarify that I never touched anything about the difficulty.

I spent 42 hours playing to get the Platinum, and this is happening to me.😢😢
