Completed Mass Effect 2 in the Mass Effect Legendary Edition on Insanity and was not awarded Insanity 2 for completing 2 mass effect games on insanity. I completed mass effect 1 on insanity and got the achievement. I started the game on insanity and played all the way through and completed on insanity. No achievement. I have started Mass Effect 3 on insanity and hopefully I'll get both achievements at the same time when I complete mass effect 3 on insanity.
I hope this bug is being urgently worked on.
Insanity difficulty is no trivial task (currently half way through ME2).
@SofaJockeyUKAlready got ME1&2 done on Insanity, it's not that bad imo in the first two games but playing ME3 on Insanity is a real struggle, too many things can one shot you :D
I got both Insanity achivements for the first two games but seeing people having problems with having it awarded is having me worried. I also had a couple other achivements that didn't get awarded to me albeit only on Steam, still got them on Origin.

How to make Insanity on ME3 easy mode (aka Garrus is now a god)
just finished ME2 and didn’t get them achievements either, bit of a bummer really
I’ve also got this issue as well as paramour 2 this needs to be fixed ASAP stuck on an earlier save until they patch this issue.
I have also had this problem along with paramour II. I saved before the relay and have completed the mission again with no luck

I'm having the same problem, but with both Insanity II and Paramore II.
Broke for me too which is hugely annoying after having spent many painful hours doing ME2 insanity.
I romanced Liara in LOTSB and didn’t get Paramour II, is that intended?
Just blew the collector base completing a full insanity playthrough of ME2.
Would be nice to get the 'badge' which didn't drop.
Will be interesting to see if a patch resolves it retrospectively or whether it will need to be achieved afresh.
