Beat all 3 games on insanity a couple of weeks ago, romanced Liara in all 3 aswell but only received Insanity and Paramour 1. Still hasn't given me the other achievements.(Playing on Xbox) trying to 100% the game so it's quite the annoying problem😅
As of July 27th the insanity and paramour achievements are still not popping. Beat the trilogy on insanity. Romanced Liara, Miranda, and Miranda. No paramour 2 or 3. No insanity 2. Please check this as beating these games on insanity can be quite the undertaking.
I have the same problem, and it's really discouraging to continue to play Mass Effect 3 with these achievements broken. And I honestly would like a refund for the game if this isn't fix yet or explain.
I just finished Mass Effect 3 Legendary (Insanity difficulty) and I did not receive my Insanity II achievement? When I imported my Mass Effect 1&2 for Mass Effect 3, I noticed the game difficulty purposely changes from Insanity to Casualty in the very beginning of the game. Which I changed right away to Insanity before I did anything else.
I hope I don't have to redo Mass Effect 3 ALL OVER AGAIN to obtain the Insanity II achievement!?
Insanity 2 didn’t pop for me after beating the suicide mission. It was set to insanity from the beginning and was never changed. I even tried reloading and replaying through the mission 2 more times and nothing. I’m on PS4 so it’s not solely tied to Xbox. This is after the latest patch.
Was it today that you finished the game?
If so give it some time, sometimes the servers work with some delay.
You can also try to power cycle your PS4.
