Was that on a specific platform?
I rescued Ash on the bomb side and had no problems whatsoever.
The Salarians and the Captain are saved either way if you destroyed the com tower, the flyers and the satellite uplink.
I don't know if it was platform specific, just remember the thread....link above if you want to refresh your memory...

It seams that is is not platform related, so the repair might work. We see.
