Yes, it seems the in-game achievements aren't synchronized, like I tried to explain in my case :
Really hope this will be fixed in a future update, because I don't see how to get Paramour II or Insanity II achievements in actual state ☹️
I've just had an update for Mass Effect Legendary Edition on my Xbox, it's only about 300mb big but after the update I checked the achievements in game and all three insanity achievements and paramour 1 was showing as unlocked, so hopefully this update has fixed it but it seems Liara in ME2 doesn't count towards Paramour 2.
Still excluding Liara from the achievement in the "legendary" edition is so fkin cheap. The only way this achievement string could have been done in a single playthrough while staying loyal.

Yep, looks like this not only fixes the achievements not triggering but credits those already achieved.
Thank you for this update ! It fixed many in-game achievements I have until then, but a last problem persist.
In my case, after finishing ME1 in insanity mode and with Asley romance (correctly got Insanity I and Paramour I) and then imported my character in ME2, finished in insanity mode too and with Miranda romance, I didn't got Insanity II and Paramour II (even Xbox achievements) and after this update, nothing has changed.
I tried to reload a save just before the suicide mission to trigger the romance scene and finish the mission, but it didn't worked.
I just started ME3, so I suppose I will get Paramour II and Insanity II achivements when finished, and I will have to replay one of the games to finally get Paramour III and Insanity III... Is there a tip to unlock Paramour II and Insanity II with my actual saves ?
It is good to see that the problem is fixed on Xbox. How about an eta on when it will be fixed for playstation
Just replayed the ME3 final mission (starting from Marauder Shields) and a save right before the Liara romance, still not unlocked Paramour or Insanity achievements. Only one this fix let me finally unlock is the one for purchasing two different armour sets.
Not sure the new patch will work for me as I did the old way of going back to the Saren boss fight to get insanity 2 once I fixed the crash issue out by turning off squad powers and not using biotic ones as it seems adding gets to the fight while a good idea those powers throw them oitside the map causing crashes
anyway guessing this will not heLip with paramour 2 if you stayed with Liara guessing I will get 2 in ME3 then I can get 3 by replaying 1
This patch did not fix the Insanity 3 achievement. I played through all three games without changing the difficulty. After downloading this patch, I loaded up ME 3 and Insanity 3 did not unlock, either in game or on Xbox. I restarted the final Citadel mission from my auto saves and replayed the entire thing. Achievement still didn’t unlock. Please don’t tell me I have to play through ME 3 again for this patch to work….
When you say the citadel fight do you mean in ME1 hope not as after I got insanity 2 by going back I did one more fight hoping to get 3 without finishing my insanity run but did not get it was hoping it was because you have to finish the 3rd game on insanity first
No, I had an auto save to restart the Citadel mission on ME 3. It put you at the point where you have to fight the 🏀 husks and the marauder and then transport up to the Citadel. I replayed all that with the Illusive Man, the kid, destroyed the reapers, watched all the final cut scenes…no achievement.
@SofaJockeyUKAh I see, yeah I've definitely got the hotfix, guess I'm just unlucky. If I gotta do insanity again I think I'm gonna be letting squadmates die haha. I am not grinding all those loyalty missions on insanity again.
EDIT: Definitely something not right for me still. I'm still not getting Paramour II either, and I literally just went back through the Omega-4 relay and re-played the romance cutscene. So that's actually encouraging because it means there's still hope for me getting Insanity II without having to do this again!
Paramour 2 unlocked for me just now. Had to romance someone other than Liara which meant having to load a save 3 missions before the suicide mission as there's 3 stages to romancing someone.
@SofaJockeyUKGot this to unlock at last but I did have to re-do a little bit. So, for whatever reason, even though Insanity and Paramour originally registered during my ME1 playthrough, reloading the cutscenes and final mission in ME2 wasn't getting those achievements to pop.
What worked for me was going back to my ME1 save and going through the Mu Relay again for the LI cutscene, and then also reloading the save I conveniently made right before the final fight with Saren and finishing that fight again on Insanity. It was like for whatever reason, for Paramour and Insanity it tracked my progress on ME1, but it didn't track that progress properly against Paramour II and Insanity II until the update, then when it was tracking properly it thought I'd only done ME2.
I wondering if because i beat ME1 before the main update (unfortunately no good placed saves) if i speed-run the game it might give me Paramore3 after the MU-cutscene and romance etc, despite having been given Paramore1 first time around, the update may of forgotten this for me. As i dont want to redo all 3 games AGAIN for 1 achievement, but I am a perfectionist.
I'm still halfway through my first play through of Mass Effect 3 but as of now I've unlocked all three Paramour achievements easily.
