Game freezes with "Active Controller Disconnected" message

As the title says, EA Sports PGA Tour has frozen on me, with a "Active Controller Disconnected" message (see attached image).  It says to "Press A on any controller to continue with that controller",...

Does this happen on a wired or wireless controller? Can you try removing PGA from quick resume, then launching the game and checking if this continues?
