Didnt get the "Completed game" achievement - All Platforms

Hi, I'm on the ps4.  I've been playing the campaign.  I'm trying to go for the campaign trophies.  But, I'm noticing that my all my story medals disappear and reset after a time.  

Is anyone else running into this problem?

I experienced this too. I also only see medals for my final mission when I go into the story mode mission selector.
I used mission selection and not the 'continue story' button to continue the story between game launches.

These include: - Completed the campaign on Story difficulty - Earned all story medals on any difficulty - Won a Fleet Battles vs AI match with both capital ships and flagship intact The campaign has also lost all of my medals between level 1 and 6. And I did not get credit for completing the challenge ‘Win a Fleet battle Vs AI’

What were you doing when the bug occurred? Step by Step detail is helpful. Finished the last mission for the Republic. I got the "Punch It" achievement but did not get the "The Galaxy's Finest (Ace)" achievement.

So I finished the campaign all on ace difficulty, and when I was sent back to the menu I noticed I didn't get any achievements for beating it. I went to check the medals for the missions and the prologue through mission 4-6 or so doesn't show any medals at all. Does anyone know what the problem is or how to solve it? And by the way yes I did go back and finish them, no medals nor achievements neither.

Same on Xbox. I have beat the game on ace, got almost all the medals in the game and nothing.

Tagging this thread with my bug report. Essentially from what I can tell, medals do not persist across game sessions. Unless you complete the game in one sitting, you won't get the achievement unless you go back and redo missions that are missing their medals.

hmm ok, I completed the first missions on Veteran difficulty, but I had to lower the difficulty after the first few missions.

Maybe this is the problem?
