Alignment control centre puzzle not unlocking

Summarize your bug The terminal that corresponds to the Chamber of Connection is not turning green and registering as completed, nor has the achievement popped. The chamber has not registered as complete even though I have collected the tear and it is marked as complete on the map. 

How often does the bug occur? Every time (100%)

What happens when the bug occurs? I cannot progress with the puzzle and collect the reward, or get the achievement. 

What do you expect to see? To have the chamber register as completed.
