This bug also affects Chapter 2, specifically the first section where the player's companions appear in a vision. None of them have their equipment (including weapons) or customizations. However, when replaying later chapters, they use their current equipment sets. I tested this with Kaliyo (Chapter 10), Jorgan (Chapter 11), Gault and Vette (Chapter 13), and Torian (Chapter 14).
Related bug: if a companion dies or leaves and you use the "transfer items to inventory" option, then replay their debut chapter, they have no items and no way to give them items. (Tested by replaying Chapter 14 on a character whose Torian died in Eternal Throne.)
