0-0-0 passive

Ok after some torture I managed to get the conq. feat done, however 0-0-0 unique seems inconstant, sometimes when attacking enemy with the debuff he doesnt gain his stack, some times he gains stack after using his special, which shouldnt happen. Sometimes things are completely normal.
I thought blindside modifier was screwing things but after removal nothing changed.
No dodges ( even though they shoudnt matter) since target was stuned, actualy thinking about it, it happened mostly when target was stuned...
