Order 66 was a surprise attack you know. Imagine walking down the hallway in the Jedi temple and suddenly your best friend attacks you out of nowhere. Or in Ki-Adi-Mundi's case being sandwiched in that bridge between clones and Droids. On top of that, some Jedi still survived. Yoda and Kenobi went ham at the temple and killed many Clones. That's why it's a chance %, not a guaranteed reflection/block.
