5* mods are from the Mod Challenge battles, which open up once you complete Table 3 of the Mods table. From there, the more tables you complete, the more challenges open up. I'm guessing that the higher tier mods will be raid rewards or special challenge rewards.
At the moment, it's interesting to see everyone has "tanked" their toons. Once the system beds in and things like the offense mods start to proliferate, I think the likes of a 40k health IG-88 will be nothing for a Leia who its for 10k basic per blast. We'll see, it's so early days and there will, I'm sure, be a period of just plain weirdness.
