I believe the same hacking methods can be used in arena, but they aren't as effective. Tournaments didn't have the same mechanism as arena where you switch places with the person you beat. It was a points system instead, and you paid ally points for the privilege of using characters multiple times (with increasing costs the more you wanted to use them). Hacking meant that people could win battles with their weakest characters and accumulate more points at lower cost. In arena, if you put in a completely junky team, you could use a hack to win and rise up, but then everybody would target you and push you back down. And there's no real reason to if you have any good teams since there's no limit on the use of characters in arena. Plus it becomes a lot more obvious and easy to report, whereas in the tournaments there was no way to see what teams people were using on offense.
Yes, fleet arena is the same as squad arena. Not necessarily the same groupings of people, but it's divided similarly based on when you unlock fleet.
ETA: It's entirely possible people are doing this in TW, I have no idea. You'd probably need a significant amount of the guild members in on it to make a big difference. The good thing there, though, is that even if you lose, the rewards aren't that much worse than if you win, depending on your bracket. With tournaments, it was an enormous difference, because the people at the top were getting a 7* character and the people at the bottom weren't even unlocking the character.
