Conquest Weak Point disk reads:
"Whenever an ally attacks, they gain +4% Offense for every debuff on all characters."
Its actual functionality appears to be:
"Whenever an ally attacks, they gain +4% Offense (stacking) for every debuff on all characters."

You are absolutely correct, the offense increase does indeed stack and therefore should be mentioned in the description. We have flagged this for correction now.
