Ok, you didn't mention that you weren't able to damage him at that max health level.
So it looks like all three things are related: Rex is dead but it somehow does not register. You can still target him, and the turret doesn't go away because he had it. GAS can't drop below 100% health because a clone is "alive". But he doesn't go into cover--that may be some other bug with losing all max protection.
