I couldn't get rid of the doll in my buy-mode, and I had the women's tops for male sims in my CAS choices. I'd tried everything here and on other posts, no luck. So I took the advice here and deleted all my .packages, DC backup, downloads (except a few tools from MTS3). I hated to lose all that stuff, but I was desperate. I deleted all my saved games too. Also, strangely enough, I had duplicates of nearly every shirt in the CAS catalog. Male, female, didn't matter, I'd have at least 6 of each. How strange? So, I deleted all the dupes from there as well.
Then I tried out a "test" sim to see if my buy catalog was cleared and it was...NO DOLL!. The female shirts for males were gone too. Don't have the strange huge boots for toddlers either.

I'm going to be a lot smarter about downloads from now on thanks to the advice here and check EVERY file before it gets near my game.
