    Uninstall your most recent EP then download and reinstall the digital version from Origin. Instructions here.

I have moved your post to this sticked thread specifically related to the disc error issues. The opening post has the links you need for instructions. 

As for the issues with Origin, you need to contact EA customer support and have them look at your account - http://help.ea.com/en/contact-us/new/
