Please no. As someone who's suffered with bad periods for many years (and is aware that many people have it far worse) and is now contemplating perimenopause I don't ever want this in my game. It's too much realism for my taste. I hate dealing with periods and related issues in real life. The last thing I want is to make my pixel people deal with it too.
I am 100% for more acknowledgement and awareness of these issues in books and TV shows, but I don't want or need it to appear in games.

FELT! The Sims is a way I often escape period misery. But I'd be fine with it as long as I could toggle it off. (Also it doesn't matter if others have it worse, your discomfort is still valid and significant! That struggle still deserves recognition)

I really don't think that any period gameplay added to the game will require a toggle. It will probably be a one day moodlet about the sim having period cramps. They will probably be able to drink some tea or take some medicine and feel better and that's about it. I am not sure how detailed the mods about periods are as I play vanilla, but it's safe to say that EA will make it to fit the teen rating if they add it.

Adding in Wonderful Whims menstrual cycles would be an EXTREMELY seriously good thing in The Sims 4.
