Did you try a new user folder? Drag “The Sims 4” folder from Documents\Electronic Arts to your desktop (don’t delete it). Then start the game - none of your saves or cc will be there. Create a new save, and test if you can play with the doctor set.
If it works in the new save, try moving back the contents of your tray folder (content you’ve saved to your library) and the saves folder. Open the game again and test in the new save - does it still work? Then switch to one of your old saves where it had been broken and test there (don’t save if it used mods). If it still works, gradually add your mods back while testing.
