@PugLove888 I removed the Mods folder to my desktop and went into my save having issues. The Elder sim was still unable to brew the potion, in the Magic Realm or on her own lot. I then created a new game and cheated up the sim to the same rank/XP perks. He learned the spell and was able to create it on his home lot. I did not try to have anyone else in the Elders save learn the potion and create it.
Another update- Cheaty MCCheatSim was able to brew a Rejuvination potion as an elder, drink it, then brew and drink an immortality potion. This was after reinstalling mods/cc.
@buddhachick, thank yo so much for the updates! 😇 Using this information I think your problem is probably a glitch with just that one Sim, or that one lot, or that one Save Game file. 🤔
There are a couple of things you can do to try to reset your original Sim in order to try to reset it so that the glitch is gone.
1). If you haven't done so already, you can just delete the localthumbcache.package file in the game folder and then repair your game through Origin (like I instructed above).
2.) You can try to travel with your household to a lot like a park, gym, or museum and then travel back. This sometimes resets Sims.
3) You can try to move out of that house and either into a new one (probably not what you want to do), or else move out and then move back into the old house. (Still tedious -- you have to remember not to sell all the furniture so it goes to their Household Inventory so they can use it again when they move back... but then you have to place everything again! 😞 I would use this as a last resort.
4). you can use a cheat to reset your Sim. Bring up the Cheat Box like before and in it type testingcheats on and press Enter
Then with the Sim you want to reset being the Sim you have selected, Shift-click on that Sim and a pie menu will pop up. Choose Reset Object (Debug) from the pie menu and your Sim should reset. (You can then type testingcheats off and press Enter when you are finished.). 😉

I’ll see if any of these work a little later today. I don’t seem to have the localthumbcache.package file. I have another for online stuff but it was empty. I’ll repair the game and see if it repopulates.
@PugLove888 I've been at it for a little while now. I DID find and delete the localthumbcache.package file. I was looking for a folder, not just a file. I repaired the game. I play tested again with the new save and it worked fine for the cheated up sims.
I've been play testing in my "glitched" save for a few hours. I had them move houses, jump lots, and used debug to reset my sim. All with no luck getting her to create the potion. I've been trying to rank up her daughter as a spellcaster to see if the glitch will effect her as well. She only has a few days left before becoming an elder... ☹️
If all else fails I can always Copypasto the potion that was created when she learned it... I haven't had her try using it since we have been trying to figure out the issue.
I agree, I think the problem is just a glitch with this save.
@buddhachick, yes, I also had problems finding the localthumbcache.package file the first time I looked for it! 😃 It is good to delete that from time to time (and necessary to delete it often if you play with Mods or CC).
Hopefully it is just your one Spellcaster that is glitched (or maybe the lot) and not the save. But weird things like this can happen, and are more likely to happen in older saves that have seen a lot of gameplay. Still, I have seen glitched Sims in brand new saves as well! 🤨
I hope her daughter can create the potion for her to drink! 🤞 But if she can't , I would consider using her Aspiration Points for an Aspiration Reward (it is found in the Aspiration Reward Store panel on the same tab as her Aspiration. It looks like a little gift icon with a bow). Then you can scroll down and get the Potion of Youth for 1,500 Aspiration Points. 🙌 It will reset a Sims life to the beginning of his or her current stage!
And the Copypasto spell on the existing potion is also a good idea! 💡

@buddhachick You could also save Cheaty McCheatSim to your in-game library, merge them into your current household, have them brew a few potions, then kick them out. It seems fair enough, since you've already done all the real work to have your elder sim brew the potion, and it's the game that refuses to cooperate.
At this point I feel like I’ve done my due diligence and tried to fix any glitches that may be around. I feel like I’ve earned some cheating on my elders behalf. I have lots of ways to make her immortal. But @puzzlezaddict @I definitely like the idea of adding my cheated spellcadter into the save game to test if he malfunctions there...
So I used Copypasto to make additional potions in case anything went awry... I had my elder use one and she has the immortality trait. Her bar is still going and I've gotten the notification to get her life in order because she's going to die... Is that normal? Is the potion not going to work? I had her also use the resurrection potion as well just in case...
Her daughter has aged to elder now but is slow on learning spells. I haven't been able to test her ability to make a potion yet.
@buddhachick, I would be careful since it doesn't look like they have fixed the Immortality potion bug yet. 😞 (I thought it had 🤭 , but I haven't tried using it since last fall. )
So I would either try it but be prepared for your elder to die, or else use the Aspiration Reward elixir that I told you about in an earlier post to put her life back at the beginning of the Elder life stage, and just take that Reward elixir any time you get the notice that she will die soon! 🤨

Edit to add: In the mean time either you can risk it with her dying, or else play a different save game file and come back to this one when they fix this Bug! Or you can make an extra save with this household by naming it something else with the Save As... button (instead of the Save button) and then experiment with playing with her, but this way you still have your original save to go back to! 😇🤞
